Referral form - new employee health screening

Please complete the following information to refer an individual for new employee health screening

Upon receipt of the completed new employee health screening referral form, we will issue the referred individual with a new employee health screening questionnaire for completion. If deemed necessary, we will contact the referred employee to complete an escalated telephone assessment. This action is required if the individual reports relevant health condition/s, disability and / or background information that may impact on fitness for work.
